Job Description

Our client is currently seeking a Medical Assistant in the Orlando, FL area!Schedule: M-F (8a-5p)This job will have the following responsibilities:1. Establishes a trusting relationship with identified patients, caregivers, clinic staff and physicians.2. Keeps patients actively engaged with their primary care physician and assists patient with any scheduling medical appointmentstransportation.3. Partners with the patients care team (community, providers, internal staff).4. Serves as primary point of contact for internal and external stakeholders.5. Provides guidance and inquires if patients need help with any specific barriers to healthcare through telephonic outreach to assist patient?s compliance with their care plans.6. Identifies health plan resources or local community resources to provide solutions for the patient.7. Engages patient, family, and caregivers to assure that a well-coordinated treatment plan is established.8. Creates referrals to outside source(s) to assist with patient?s self-management goals.9. Supports the patient to ensure pick-up and adherence of their medications.10. Proactively engages the patient to manage their care.11. Liaisons with providers and facilities telephonically, to assist in problem solving regarding patient needs.12. Communications with internal staff, patientcaregiver and community providers must be accurate and timely.13. Patient outreach using compassionate customer service with the goal to inform them of care gaps and facilitate the closing of those gaps.14. Documents all patient interactions in a concise manner that is compliant with documentation requirements for Model of Care and Center for Medicare and Medicaid Services (CMS) regulations.15. Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, surveysscreenings, census management and distribution of materials to appropriate clinical personnel or patients.16. Provides clerical andor administrative support to clinical staff and managers.17. Enters timely and accurate data into the electronic medical record to communicate patient needs and ensure complete documentation of patient visits and phone calls.18. Performs all duties for internal and external customers in a professional and responsible manner.19. Adheres to organizational and departmental policies and procedures.20. Performs all other related duties as assigned.Qualifications & Requirements:High School Diploma or GEDTwo or more years? experience in a physicians office, clinical or hospital setting.Experience working with psychiatric and geriatric patient populations.Excellent verbal and written skills.Ability to interact productively with individuals and with multidisciplinary teams.Proficient computer skills, including Microsoft Word, Excel, Access and Outlook.Excellent organizational and prioritization skills.If interested, please apply directly or reach out to one of my contact methods below ASAPCALLTXT: (484) 753-3515EMAIL:
