Previous experience in care coordination or a related role in the healthcare industry. Strong organizational, communication, and interpersonal skills. Knowledge of home care regulations and industry best practices. Ability to handle and resolve conflicts effectively. Proficient in computer applications, including Microsoft Office and care management software. CPR and First Aid certification. Passion for providing outstanding care to clients.Preferred SkillsClinical Expertise: Proven experience in providing direct client care and ensuring safety. Ability to develop, implement and evaluate organizational and client-focused documentation in the system. Good understanding of the government funding environment for aged care (All levels of Home Care Packages). Good understanding of the hospital discharge environment. After any interactions with hospitals or health professionals, follow up as needed. This may involve scheduling follow-up appointments, obtaining test results, or clarifying instructions. Having a good understanding of how to liaise with hospitals and health professionals is a must. Good understanding of health assessments and developing care plans to meet clients'' care needs.
