Looking to join a team coordinating care and services to support patients in their own home? We have a opportunity available to work within the SA Community Care (SACC) and the My Home Hospital (MHH) program.Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our clients together with:Conducting clients initial phone assessment, care planning and goal settingCoordination of in-home services to support clients receiving care at homeBuilding and maintaining effective relationships with a variety of different stakeholdersMonitoring patient progress and updating care plans as appropriateLiaison with healthcare professionals and specialistsDischarge planning including referrals to long term servicesProvision of telephone support, education, and advice.Our Care Coordinators work a rotating 7-day roster in the comfort of your own home.Please note that this role is based in Adelaide (South Australia) and we offer great flexible working options to achieve a better work-life balance for you & your family.What will set you apart from the rest?An experienced clinician, you are customer focused and driving to deliver best practice outcomes across our programs.
