Our range of services includes Supported Independent Living (SIL), Home and Community Support, Support Coordination, and Community Nursing.Role DescriptionWe are currently seeking an enthusiastic and experiencedService Coordinatorto join our growing team. This vital role oversees the delivery of personal care, community access, and 24/7 SIL supports, ensuring our services operate smoothly and in line with NDIS standards. You will be a hands-on leader, guiding and supporting our team of Disability Support Workers, while fostering a workplace culture built on trust, respect, and collaboration.Key ResponsibilitiesIn this dynamic role, your day-to-day responsibilities will include:Overseeing and coordinating the daily operations of Altitude Cares support servicesLeading, mentoring, and supervising support staff to ensure high-quality, person-centred careLiaising with internal and external stakeholders to maintain consistent service deliveryFacilitating participant reviews, team meetings, and performance discussionsManaging rosters, approving timesheets, and ensuring adequate shift coverageEnsuring all documentation and service records meet NDIS compliance standardsConducting staff recruitment, onboarding, training, and ongoing developmentOverseeing incident management, WHS procedures, and risk mitigation strategiesParticipating in the on-call rotation and addressing operational matters as they ariseContinuously identifying opportunities for service improvement and implementing best practicesQualificationsCertificate IV or higher in Disability, Community Services, or a related fieldDemonstrated experience leading teams in the Disability or Community Services sectorSound knowledge of the NDIS and its Quality & Safeguarding FrameworkExcellent communication, interpersonal, and time management skillsStrong administrative and coordination capabilitiesA person-centred mindset with a deep respect for participants rights, dignity, and goalsValid Driver Licence, NDIS Worker Screening Check, Working with Children Check, current First Aid Certificate, and legal right to work in Australia.What We OfferOngoing professional development and training opportunitiesSupportive team environment and strong organisational valuesA meaningful role where youll truly make a difference in peoples livesReady to Join Us?If you are passionate about empowering others and meet the qualifications above, wed love to hear from you.
