Collaborating with patients, families, physicians, and the interdisciplinary team, the role ensures seamless care from acute to post-acute settings. The position involves leadership in care coordination, promoting optimal clinical outcomes, and effective resource utilization through monitoring hospitalizations and managing utilization review activities.Minimum QualificationsBachelors of Science in Nursing (BSN)Two (2) years recent experience in clinical, utilization management, discharge planning, or case management rolesSkills and CompetenciesHigh clinical competence and knowledge of disease processes and treatment protocolsEffective communication skills with diverse healthcare stakeholdersAbility to organize, prioritize, and problem-solveDemonstrated professionalism and accountabilityUnderstanding of patient populations'' needs for dignity and respectPhysical DemandsSedentary exertion, up to ten pounds of force occasionallyCompensation and BenefitsHourly rate: $33.66 - $50.49, based on experience and location.
