Position Title: Manager at Home Programs Employment Type: Ongoing full-time Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028 Role description The Manager At Home Programs is a Senior Leader position that sits within the Community Services and Wellbeing Division within our Continuing Care Directorate working closely with the Community Manager group and other key Senior leaders to ensure optimal management of resources to facilitate high quality person centred care. The Manager At Home will oversee and provide strategic program, operational leadership and management support to the following suite of services and roles: Allied Health Inpatient service provision and the Transition Care Program (TCP) Allied Health Program Leadership for Allied Health in Residential Aged Care and the Allied Health Assistants At Home Programs including Hospital in the Home (HITH), Residential In Reach (RIR) and the further establishment and expansion of the Geriatric Evaluation and Management (GEM) At Home and Rehabilitation in the Home (RITH). This role is a key leadership position that supports the organisation to increase the care options available for patients, support patient flow initiatives, and enable patients to access the right care, in the right place at the right time. With a focus on continuity of care and multi-disciplinary care provision that enables care in the home and improved patient experience and optimal health outcomes. As a leadership role there is a strong focus on providing seamless care transitions for people to support recovery of function, independence and wellbeing, and promote self-management of acute on chronic conditions. Primarily based at our Castlemaine campus, but will be required to work across both campuses and across the diverse range of programs including the interface between the acute bed based and aged care residential care services to support access, flow and continuity to the ambulatory care and at home services. About you Bachelor of Nursing with current AHPRA registration Minimum of 10 years'' experience working professionally in a relevant clinical area Excellent organisational skills with a proven ability to manage multiple tasks and work to deadlines Current valid drivers'' licence Relevant post graduate experience in leadership and management will be highly regarded.
