About us The newly integrated Adult and Older Adult Service combines AMAH Adult Community Mental Health and Caulfield Aged and Liaison Psychiatry Services to provide comprehensive, person-centred support for adults and older adults. We offer a wide range of therapeutic, addiction, trauma-informed, and family violence services. In line with the Royal Commission into Victorias Mental Health recommendations and the Victorian Mental Health and Wellbeing Act 2022, we are expanding our services to include pathways for substance use, rehabilitation, recovery, and trauma-focused care. Our approach emphasizes integrated treatment, education, peer support, care planning, and navigation, supporting individuals at every stage of their recovery journey. Areas of work include: Navigations and Continuing Care: Brief Intervention, Access and Intake, CMHT, Physical Health, and Family Services Adult Rehabilitation and Recovery: MSTS, CCU, PARC, HOPS, and SECU Aged/Older Adult Mental Health: MAPS, Caulfield Hospital Community Liaison, and inpatient units Wellbeing and Therapeutic Services: Psychological Trauma and Capacity Building initiatives Join us in reshaping mental health care with compassionate, specialized support to create meaningful change for our community. About the Role The Senior Mental Health Practitioner will provide discipline leadership, supervision, and support within a designated team. They will work with an allocated client list, providing clinical case management, treatment, and support services, as well as discipline-specific input to the multidisciplinary team. The successful applicant will also contribute to the development of their discipline across Alfred Mental and Addiction Health.
