Aboriginal Health Rural Generalist Nurse Practitioner (RGNP) - Identified - CoffEmployment Type: Permanent Full TimePosition Classification: Nurse PractitionerRemuneration: $139,661.16 - $149,538.56Hours Per Week: 38Requisition ID: REQ566662Advertising Closes: 5th May 2025Employment with Mid North Coast LHD requires candidates to hold a valid authority to work in Australia (Aust Work Rights Visa).What you''ll be doingThe Rural Generalist Aboriginal Health Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) provides high-level, clinically focused care across emergency, inpatient, chronic, complex, and primary care services at specific sites.Provide holistic care for a broad range of acute and chronic presentations to meet patient and community needs.Facilitate integrated care through referrals, contribute to clinical excellence via research, teaching, consulting, and leadership within the nursing profession.Work autonomously and collaboratively in an advanced nursing role, managing clinical demands within their scope of practice, including chronic and complex cases.Offer expertise in chronic and complex health conditions, supporting best practice emergency/inpatient care plans and discharge planning for high-risk Aboriginal and Torres Strait Islander patients with medical diagnoses at Coffs Harbour Health Campus and Galambila Aboriginal Medical Services.Develop culturally safe partnerships to address priorities within the Coffs Clinical Network community, including Aboriginal and Torres Strait Islander peoples, people with disabilities, those experiencing homelessness or on low incomes, older adults, and culturally diverse groups.Coordinate healthcare activities and referrals, liaising with community agencies and services relevant to patient conditions.The NP/TNP role is grounded in nursing values, knowledge, theories, and practices, delivering innovative, flexible healthcare that complements other providers.
