Key Responsibilities Conduct client assessments and develop tailored care plans Manage and support a caseload of clients across the Gold Coast North and South regions Ensure compliance with all relevant care standards and funding guidelines Coordinate personalised services in line with care plans and budgets Provide administrative support to ensure smooth service delivery Foster strong relationships with clients, families, and external providers What Youll Need Relevant qualification (Diploma in Nursing) Previous experience in aged care, NDIS, or mental health case management Sound understanding of My Aged Care, Home Care Packages (HCP), and NDIS frameworks Strong skills in assessment, care planning, and client coordination Proficiency in Microsoft Office 365 and Outlook Excellent communication and customer service skills Professional phone manner and strong problem-solving abilities Ability to work both independently and collaboratively Commitment to privacy, confidentiality, and ethical care delivery Why Work With Us?
