Job Opportunity: Outreach WorkerWe are recruiting a full-time Outreach Worker (37.5 hours, 12-month contract) at the Irish Support Agency (ISA), NSW. This role is vital in maintaining and expanding our impact within the Irish-Australian community, providing support to individuals experiencing vulnerability or distress.Responsibilities include:Providing high-level support to clients.Conducting person-centred assessments and care planning.Ensuring support delivery aligns with policies and funding requirements.Collaborating with other agencies for holistic service delivery.Advocating for clients with government and non-government organizations.Participating in interagency case meetings.Visiting clients in various settings such as nursing homes, hospitals, detention centers, and prisons.Helping clients navigate aged care supports and services.Attending and contributing to agency-hosted events.Ideal Candidate:Understanding of Irish culture within the NSW and Australian context.Flexible, proactive, and solution-focused attitude.Knowledge of community, social, and health services and referral pathways.Proficiency in computing skills.Excellent interpersonal, written, and verbal communication skills.Current NSW drivers license and own vehicle.Remuneration will be based on qualifications and experience. #J-18808-Ljbffr
