Occupational Therapist - Community Aged CareJoin to apply for the Occupational Therapist - Community Aged Care role at Sublime.Ongoing, Full-Time or Part-Time (min. 3 days)Remuneration aligned with NSW Health LevelsSouthwest Sydney Location - work in community and from homeThe RoleYou will be responsible for managing a caseload of participants aged 65 and over, supporting them in maintaining their independence at home. You will prescribe home modifications and equipment and develop personalised treatment plans based on assessments and client goals.
