The role utilizes the Transition in Place housing model to facilitate rapid exit from shelter, employing evidence-based practices such as Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Housing First.ResponsibilitiesDevelop and implement individualized service plans to help clients maintain housing and achieve personal goals, including mental health, substance use management, income increase, and self-sufficiency.Conduct recertifications, home visits, and follow-ups to support housing stability and independence.Promote a culture of diversity, equity, and inclusion through anti-racist practices across the department and agency.Job DetailsThis is a non-exempt, union position with a starting wage of $20.00/hour.
