All services are delivered by fully qualified, interviewed, and screened carers who undergo regular performance reviews, ensuring continuous improvement and high-quality care.Role DescriptionThis is a full-time hybrid role for a Case Manager, located in Penrith, NSW with some work-from-home flexibility. The Case Manager will be responsible for coordinating and managing in-home care services for clients. Daily tasks include assessing client needs, developing and implementing care plans, facilitating access to funding, managing documentation and paperwork, and monitoring the delivery of services.
