See more at our website - www.atsichsbrisbane.org.au Description About The Role The Youth Practitioner provides tailored case management services for young people at risk of disconnecting with their family/community, disengagement from education, training and employment, homelessness, or at risk of harm. They are responsible for engaging effectively with young people, their families, and support networks to identify needs and aspirations, and to develop and implement a plan led by the young person. Duties Meet individual case performance targets as per weekly, monthly, and quarterly dashboard system. Provide high-quality, intent-based practice in all interactions, aligned with ATSICHS values and vision. Maintain high-quality case management practices and record keeping according to operational requirements. Engage clients through an assertive outreach approach, demonstrating warmth and positivity. Facilitate client-led assessments and Care Plans to identify goals, including family involvement where appropriate. Implement targeted interventions and facilitate family group meetings to achieve client goals. Ensure high-quality management using LogiQC systems and MobNet tasks. Skills and Experiences COVID-19 Requirement Immunisation history showing two doses of COVID-19 vaccine is encouraged. Required Current Diploma or actively studying towards a Diploma in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or 2 years relevant experience. Ability to work in a performance-driven environment to meet targets. Ability to build client agency and facilitate change through empowerment. Knowledge of behaviour change interventions and family needs. Experience in coordinating multiple services for holistic client care. Understanding of factors contributing to vulnerability and trauma. Experience with case management for high-risk/crisis clients. Knowledge of support services in mental health, substance misuse, education, employment, and housing. Additional Requirements Travel across Logan and/or Brisbane regions. Proof of work rights in Australia. Current drivers license. Work 1-2 night shifts weekly (not every week).
