Our aim -to make Orange a better place to live today and tomorrow.About the roleThe Choices at Home Team Leader is responsible for leading the Choices at Home team to deliver community support activities which help older people live as independently as possible at home, connected to their community. This will include:Planning the delivery of choices at home services to older people in line with contractual and budgetary requirementsCoordinating the delivery of Commonwealth Home Support (CHSP) ServicesBuilding community awareness of Choices at Home services and increasing usage by seniors seeking entry level CHSP servicesNetworking with local NGOs, community organisations, medical centres and allied health specialists to raise the profile of services offered by Choices at HomeUndertaking client assessment, making appropriate referrals and identifying service gaps by researching the needs of the target groupWhat can we offer?Flexible working arrangements including rostered days off and variable hours.Health and Wellbeing benefits including access to a Fitness Passport for you and your family.Salary Packaging available including novated leasing.A range of in-house and external training opportunitiesFree access to EAP services for you and your familyOrganisation wide health and wellbeing initiativesSo what do you need?To be successful in this role you will need:Diploma of Community Services or equivalentDemonstrated experience leading a teamExperience in planning and delivering services to older peopleHighly developed organisation and administrative skillsCurrent Drivers LicenceFirst Aid CertificateAbility to obtain a satisfactory criminal records checkTo apply, please click Apply.
