The QIDP''s primary function is to develop, integrate, coordinate, and monitor the implementation of the Individual Service Plan (ISP) to ensure quality services.POSITION RESPONSIBILITIES:Quality assurance & training - Coordinates and trains Direct Support Professionals in implementing individual habilitation plans. Monitors and evaluates service provision, providing training on plans and general support to promote continuous quality improvement.IHP development & problem solving - Evaluates each individuals needs, strengths, preferences, and abilities, ensuring plans are current and relevant. Communicates with team members to identify and resolve emerging issues.Team facilitation - Acts as a facilitator during Interdisciplinary Team Meetings, including admissions, reviews, and habilitation meetings.Guardian communication - Engages guardians as active team members, building trust and obtaining necessary consent.
