The Primary purpose of the role is the provision of trauma informed assessment and intervention which promotes, protects and maintains the health of children/young persons in Out of Home Care.The Primary purpose of the Social Worker (Lvl 3) role is the provision of trauma informed assessment and intervention, which promotes, protects and maintains the health of children and young people in OOHC. This includes assessing, reviewing and developing Health Management Plans, which address the physical, developmental, psychosocial, sexual and mental health of children and young people in OOHC. Delivery mode is to be adaptable and flexible according to need and resources available.The Social Worker (Lvl 3) will provide professional and carer education around health needs of children in OOHC. The Social Worker (Lvl 3) will provide specialist social work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.Where You''ll Be WorkingSWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. View our application guide for information on how to respond to criteria and improve your application.Social Work degree which provides eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.Extensive post graduate experience in working with children, young people (0-18 years) and families within the Child Protection context.Demonstrated knowledge of Child Protection Legislation and policy and procedures, with a focus on Out of Home Care.Demonstrated advanced level of skills in complex psycho-social assessment and extensive specialist psycho-social knowledge of children in relation to: child protection, substance misuse, mental illness, domestic violence and the impact this may have on attachment and development.Demonstrated ability to create and maintain close working relationships with NSW Health Services, Department of Communities and Justice and Non-Government agencies, and multidisciplinary colleaguesDemonstrated high level communication skills (written and verbal) and demonstrated computer skills.Demonstrated ability to prioritise work and manage stressful situations; ability to work unsupervised and apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature.Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated capacity to provide clinical supervision.Applicants must possess an Australian C-Class driver''s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
