Join a team that cares as much as you do.About the role:Were seeking a passionate and organised professional to join us on a 9-month maternity leave contract (with the possibility to extend), supporting our Aged Care clients as a Care Partner (HCP).The successful candidate will initially step into the role of Aged Care Support Advisor, supporting our Home Care Package (HCP) customers with personalised, high-quality care coordination.With the national transition to the Support at Home Program in July 2025, this role will evolve into the newly created Care Partner position offering the opportunity to be part of an exciting reform and play a key role in shaping how care is delivered into the future.In this role, you will:Assess clients needs and develop tailored care plans.Coordinate services and work with families, staff, and health professionals.Review and adjust care plans as needs change.Empower clients to understand their rights and available supports.Monitor budgets and ensure services are delivered in line with funding and care goalsEnsure services are delivered safely and meet quality standards.Advocate for clients'' rights and ensure they receive necessary support.Collaborate with internal and external teams for seamless care delivery.Monitor service delivery to maintain high standards and address any issues.Provide ongoing education to clients and families about available services and funding options.Keep accurate and up-to-date documentation for all client care.About you:Youre driven by purpose, with a strong foundation in care coordination and a flexible, team-first attitude.
