The role will ensure engagement regarding immunisation and health screening, and broader preventative care initiatives, within and across the region is current and relevant to primary and aged care.Key components of the role are to:Work with key Adelaide PHN staff to develop local, cost-effective and innovative projects to improve immunisation and screening rates in the region, integrating with current initiatives.Monitor and report on current issues in preventative care initiatives, including immunisation and screening, to ensure Adelaide PHN is cognisant of the unique needs of the community and local providers within the region.Foster and maintain effective relationships with a broad range of stakeholders delivering immunisation and health screening programs and services.Enable engagement, education and communication strategies with key stakeholders to support quality immunisation and health screening programs/services.Lead engagement with General Practice and Residential Aged Care Homes to improve population vaccination and screening uptake.Support Residential Aged Care Homes to plan and deliver 6 monthly COVID 19/Influenza vaccination clinics, (where low levels of uptake are identified).This is a rewarding role requiring a person who is passionate about improving health outcomes; has a track record of successful project delivery; enjoys learning and wants to work with like-minded people who contribute to improved health care in the community.
