The role involves providing comprehensive, patient-centred care to individuals with diabetes and their carers, and educating team members about diabetes management within the Hospital in the Home program (HITH).This is a full-time, ongoing position, Monday to Friday, 8:30 am 5:00 pm.Key Qualifications and Skills:Experience with Type 1, Type 2, and Type 3c diabetes management.Ability to assess, plan, and implement individualized education programs for patients and families.Familiarity with diabetes technologies such as Continuous Glucose Monitors and insulin pumps.Ability to work autonomously and collaboratively within a team, with strong problem-solving skills.Experience working with clients from diverse cultural and linguistic backgrounds.Current credentialing with the Australian Diabetes Educators Association or working towards it.Next Steps:If interested, please click the ''Apply'' button and follow the prompts.
