Our multidisciplinary team empowers and advocates for healthy rural communities through providing diverse and excellent health services throughout country South Australia. Choice of Location - Port Lincoln or Whyalla Flexible working arrangement (Full-time or Part-time)The RoleAs a NDIS Support Coordinator, you will work closely with participants to maximise the usage of their plans to support building independence and progressing towards their goals. You will be pivotal to linking participants with appropriate services and providing high quality person centred support.About youYoure passionate about empowering people with disabilities to achieve their goals and live more independently. You bring experience from the disability, community, or mental health sector, and are known for being compassionate, resilient, and a natural problem-solver. Your strong communication skills help you build meaningful and trusting relationships with participants, families, and service providers. Youre confident using Microsoft Office and have experience with CRM platforms, which helps you stay organised and responsive in a fast-paced environment.Whats on offer?
