This is a full-time, 12-month fixed-term role with the potential for extension. Based in Grafton and/or Coffs Harbour, the position supports participants across our wider footprint. If you are ready to make a meaningful impact and be part of something bigger, this could be the perfect opportunity for you.About the RoleAs a Support Coordinator, you will walk alongside people with a disability, helping them to make informed choices by connecting them to the supports they need to achieve their goals under the NDIS.In this role, you will:Support participants to understand and implement their NDIS plans, promoting informed choice and control.Build participant capacity to navigate systems, coordinate supports, and work toward their personal goals.Facilitate access to services, providers, and community networks that align with each participants needs.Strengthen informal and mainstream supports to foster inclusion and community participation.Identify and manage service barriers and risks, escalating complex matters when necessary.Develop and maintain strong relationships with service providers and stakeholders to ensure coordinated, holistic support.Promote a safe work environment by complying with Work Health and Safety (WHS) policies and reporting obligations.Maintain accurate and up-to-date records in accordance with organisational, legislative, and NDIS requirements.About YouYou are a compassionate, self-motivated professional who thrives on helping others succeed.
