Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia.This fixed term full-time (12 months) role is based at our Spring St office in Melbourne, with the flexibility to work from home a few days a week.The day to dayCoordinating and managing acute Hospital Care at Home programs for clients discharging early from hospital to home-based careSupporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environmentImplementing, monitoring and modifying in-home care plans to meet client needsManaging clinical case escalations and variations to expected clinical progressSupporting clients to achieve their personal and clinical goals within their home environment.Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers and fundersFlexibility for Work-Life BalanceExtra Well-Being Leave daysFitness Passport DiscountsBuy up to 2 Extra Weeks LeaveEmployee Discounts on Banking, Health, InsurancesNovated Leasing for a New CarAbout YouQualification as an Allied Health professional or Registered Nurse with current AHPRA registration / Exercise Physiologist with current ESSA accreditationExperience working in a multidisciplinary collaborative teamExcellent knowledge of hospital discharge planningAbility to work in a dynamic environment with tight timeframes and competing prioritiesExcellent clinical reasoning and problem-solving skillsExcellent verbal and written communicationsStrong administration and time management skills, tech savvyPassionate about health and committed to making a real difference in the communityTake the next step in your career by clicking APPLY or contact Anna Mitchell at AnRichards@australianunity.cm.au.
