The Role We are seeking an experienced Team Leader with a clinical background in aged care to lead our Single Aged Care Assessment team. This role involves overseeing assessors statewide, managing referrals, and ensuring high-quality service delivery through quality management processes. Responsibilities Manage key performance indicators for Aged Care Needs Assessments as per contractual obligations. Lead the assessment team to deliver comprehensive in-person and remote assessments. Develop strategies to connect clients with community services, empowering their independence. Engage in peer review processes with triage and assessment teams. Assign and reassign referrals for triage and assessments. Support and mentor assessors in the field. Essential Requirements Tertiary qualifications in a health-related discipline (e.g., Nursing, Social Work, Occupational Therapy). Minimum two years of leadership or supervisory experience in aged care or a related field. Knowledge of Home Care Packages, Restorative Care, Transition Care, and Respite Services. Experience working in KPI-driven environments to ensure quality outcomes. Unrestricted Australian Drivers Licence. National Police Check, NDIS Blue and Yellow Cards, First Aid and CPR certifications.
