We are looking for an experienced Team Leader with a clinical background in aged care to guide our assessment teams.The primary purpose of the Aged Care Assessor Team Leader is to oversee the activities of our assessors state-wide and manage incoming referrals. This role will enforce stringent quality management processes to drive continuous improvement, guaranteeing consistent excellence in service delivery.Responsibilities:Manage the Aged Care Needs Assessments key performance indicators as contractually directed.Lead the assessor team to deliver high-quality in-person and remote comprehensive assessments.Develop and monitor strategies for assessors to connect people with appropriate services and supports in their community to empower clients and enhance their independence. Engage in peer review processes with triage and assessment delegates and assessors.Assign, un-assign, and reassign referrals for triage and assessments.Provide ongoing support and mentorship to assessors in the field.Essential Requirements:Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or similar).At least two years of experience in a leadership or supervisory role in aged care or a related field.Sound knowledge of Home Care Packages, Short Term Restorative Care Program, Transition Care Program, and Residential Respite Services.Previous experience performing in a KPI environment to ensure quality client outcomes.At least one year of experience in a related role is preferred.Unrestricted Australian Drivers Licence.National Police Check.NDIS Blue Care and Yellow Card.First Aid and CPR certifications.Proof of right to work Australian Birth Certificate, Passport, citizenship, or Visa.What we can offer you!Opportunity to maximise take-home pay through salary packaging benefits of up to $18,550.Paid mileage and travel time for work-related travel between client visits.Hybrid/flexible working arrangements.Q Leave entitlement.Fully paid training to ensure you have the skills to thrive in your role.Access to ongoing professional development opportunities to grow your career.
