Supportive and collaborative team environmentPermanent part time 4 days a week Day shifts only - Monday to Friday - no late nights!Sunny Gold Coast Location - Close to public transport (tram and bus)Gold Coast Private Hospitalare seeking an experienced Registered Nurseto join the team as a Discharge Coordinator in thispermanent part-timeopportunity.Must be available a minimum 32 hours a week across 4 weekdays to suit the roster.Day shifts only, typically 6.30am start however flexibility of start time can be considered for the right candidate.About usEstablished in 2016, Gold Coast Private Hospital is a state-of-the-art, 339-bed facility located in the heart of the Gold Coast Precinct in Southport.With 22 operating theatres, Gold Coast Private Hospital offers a wide range of specialties including, but not limited to, 24/7 Emergency Care, Medical Services, Orthopaedics, Bariatrics, Neurosurgery, Plastic Surgery, ENT (Ear, Nose, and Throat), General Surgery, Obstetrics, Cardiac Care, and Oncology.Gold Coast Private Hospital is committed to continual growth, providing exciting opportunities and professional career pathways in healthcare.AsDischarge Coordinator your responsibilities will include:Being responsible for providing optimal discharge planning in line with the strategic direction, operational processes, budget and business direction of the hospitalBeing responsible for reporting clinical activities and revenue matters to the Allied Health Manager and the hospital Executive TeamDelivering seamless patient flow, effective bed utilisation and episode managementLiaising with the multidisciplinary team including Nurse Unit Managers, Nursing Staff, Allied Health Professionals and Medical Practitioners to coordinate patient discharge plansLiaising with external service providers for patients with ongoing care needs post dischargeSupporting the patient and family by providing appropriate information and options regarding health and nursing care and facilitating acceptable discharge plansForward planning and early screening of inpatients with complex care needs to facilitate timely dischargeEnsuring information relating to patient discharge plans is documented and accessible by all clinical staffSelection Criteria:Extensive experience as a registered or Enrolled nurse in an acute care facilityKnowledge of My Aged Care, NDIS, TCP, QCAT & ACAT is essentialMust be organised and efficient with outstanding time management skills and effective prioritisation skills Must be highly skilled with LOS (length of stay) management and patient flow including meeting LOS KPIs and working with both clinical and revenue teams to optimise outcomes Must have strong communication skills including the ability to have direct and effective conversation with patients, care facilities and other key stakeholders such as family/ carers etc.Able to generate referrals to relevant organisations, establish relationships with key stakeholders and network to grow connections in relevant facilities/ servicesAbility to support and liaise with respective NUMs and Departments and report on actions, outcomes and activityAbility to problem solve in a fast-paced environmentAbility to work autonomously and within a team environmentHealthscope, where career means more!No matter where you are at in your career, at Healthscope you can choose your own adventure.
