This means we encourage Aboriginal and Torres Strait Islanders, people with disabilities, LGBTIQ+ and from culturally diverse backgrounds to apply for this job, with age being no barrier.Job DescriptionThe opportunity:We are looking for a Healthy Ageing Service Access Partner to join our supportive and professional multidisciplinary teams in a full time capacity.Responsibilities include but are not limited to;Providing quality care partnership services by embracing contemporary practices and ensuring compliance with Aged Care Quality standards.Communicate program and service offerings to new service participants.Explain service agreements, including participant rights and responsibilities, to support informed decision-making.Provide clear information on co-contributions, fees, budgets, and service availability.Gather and document initial participant information based on individual needs, eligibility, and preferences; develop person-centred service plans in collaboration with participants or their nominated supporters.Coordinate the onboarding process, including finalising service agreements and contracts.Liaise with and advocate to aged care providers and key internal and external stakeholders to support participant access to services.Ensure services are responsive and culturally appropriate to the diverse needs of participants and their carers.Conduct risk assessments collaboratively with participants and carers and implement strategies to address identified risks.Manage participant and carer complaintsTravel within the community to meet with participants and attendance at promotional events is also required.Desired Skills and ExperienceExtensive experience working with older people and providing care management and coordination.Strong understanding and application of Consumer Directed Care (CDC) principles, supporting participant choice and preference.Sound knowledge of specialist aged care services, including support for Parkinsons and Dementia.Familiarity with aged care reforms, including the transition to the Support at Home ProgramUnderstanding of the Aged Care Quality and Safety Standards, the Aged Care Act, and the Victorian Health Records Act.Knowledge of the Aged Care Single Assessment process, including assessments for residential and permanent care services.Advanced skills in planning, organization, and time management.Strong interpersonal and communication skills, with a demonstrated commitment to person-centred care.Experience working with vulnerable and special needs groups, including people from CALD and/or Aboriginal and Torres Strait Islander communities.Working knowledge of program-specific operational manuals and compliance requirements.Please apply here by submitting a cover letter addressing the key selection criteria with your CV.
