You will ensure that individual plans align with participants'' passions and interests, promoting their quality of life, while also building the capacity and strengths of their support networks.Successful candidates will provide support to NDIS participants living in the NSW Central Coast region.Responsibilities include:Assisting participants and their families/carers to identify needs through assessment and person-centred planning.Developing and delivering evidence-based support plans.Creating meaningful goals and strategies within a positive behaviour support framework, with a commitment to eliminate restrictive practices.Ensuring plans reflect participants'' goals, passions, and strengths, helping them get more out of life.Building the knowledge and skills of support networks, including family and carers.Acting as an advocate and role model for best practices.Collaborating within a multidisciplinary team.What''s in it for you?We support you as you support others!
