We strive to ensure that every young adult has the resources and opportunities needed to lead fulfilling lives.About the RoleGympie''s Children, Youth and Families are providing a service as part of its Next Step Plus and Extended Care Program and are seeking a Full-Time Transition Mentor - Out of Home Care to work 76 Hours per fortnight however hours may increase. The role will be based from our Gympie office.As a Transition Mentor, you will play a pivotal role in providing case management and support to young adults transitioning out of care and post-care.
