Pathways to Good Health is a multidisciplinary health care service for children and young people who have entered out of home care. The service aims to provide these children and young people with health management plans that meet their unique needs in a timely manner.As the Aboriginal Health Pathways Worker, you will be responsible for assisting children/young people and their families to navigate the service and break down barriers to involvement with the service. You will also be responsible for liaising and relationship building with VACCA and local First Nations Health care providers.This is a .4 EFT fixed term role until June 2027. This position works across our Werribee site with VACCA and Caroline Springs and Melton campus of Western Health.Key responsibilities:Identify and navigate barriers to children and young people accessing health care.Liaise with multiple external services while providing service navigation.
