This position is responsible for providing comprehensive case management and connections to housing resources and supportive services to assist clients transitioning out of homelessness. This position aims to empower homeless individuals with the necessary tools to achieve housing stability and increase income, aligning with the overarching goal ensuring homelessness is a brief, infrequent, and one-time experience for those served.Some Of The Duties IncludeCollaborate with clients to devise Exit Plans that accelerate their transition out of homelessness, connecting them with suitable destination options based on eligibilityFacilitate referrals and connections to community partners, as appropriateAssist shelter clients in enhancing their income prospects by guiding them towards job training programs, employment opportunities, or facilitating SSI/SSDI applicationsAlign clients'' trajectories with their Exit Plans by coordinating their engagement with other Communities within the ShelterEnsure clients integration into Cuyahoga County''s Coordinated Entry systemArrange client transportation to critical destinations outlined in their action plansAdapt to fluctuating needs by floating between Communities, as neededMaintain participant data and statistical information in the client database systemEngage in regular caseload reviews with supervisor(s)Assist in managing Shelter emergencies in adherence to program policies, prioritizing the safety of both clients and staffConduct bi-weekly reviews with each community participant to monitor progressApply best practices in case management (e.g., Motivational Interviewing, Critical Time Intervention) and shelter operations (e.g., Housing First, Low Demand, Harm Reduction).QualificationsOur ideal candidate has a High School Diploma or GED; Bachelors Degree preferred. Proven experience identifying, accessing, and coordinating with community partners and social service resources; experience working with vulnerable populations and people with lived experience; and the ability to uphold and support the Housing First Philosophy.LMM offers a number of benefits for full time staff including, but not limited to: medical dental, vision, disability, life, generous paid time off and a 403 (b) retirement savings plan.
