In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.The Role:The Grade 2 Physiotherapist will provide leadership and ensure high level, evidence-based physiotherapy care to patients managed in the Heart Failure Virtual Ward and Heart Failure Rehabilitation across Northern Health. This program aims to move heart failure patients from an in hospital inpatient admission to hospital in the home under the care and monitoring of the Heart Failure team.
