The role involves expanding primary functions of pharmacists such as providing individualized health promotion and disease prevention, including vaccine administration, long-acting HIV medicine administration, diabetes monitoring, nicotine reduction programs, and other subject matter expertise, in collaboration with other AHF providers.A Clinical Pharmacist - SRT enhances the reputation of AHF by improving patient outcomes, expanding current services, creating opportunities for professional growth and satisfaction, and supporting the development of the AHF Healthcare System.You will be a passionate advocate for our top initiatives.To succeed as a Clinical Pharmacist - SRT, it is crucial to uphold AHF''s core values and mission in all your actions. At AHF, we are nimble and adaptable in a dynamic environment to provide the best experience for our clients and workplace for our employees.AHF has a collaborative organizational structure where staff are accountable to multiple leaders.The Clinical Pharmacist - SRT will work as part of a close-knit team to provide health promotion and disease prevention, including immunizations, diabetes and smoking cessation programs, as well as other subject matter expertise programs developed by AHF, with appropriate collaborative drug therapy management authority. This includes prescriptive authority where legally and organizationally permitted, such as PrEP and PEP therapies.Licenses/Certifications: Additional state licenses may be required based on organizational needs.
