In this pivotal leadership role, you will:Key ResponsibilitiesOversee resident assessments and develop tailored care plansCoordinate clinical documentation and compliance reporting (AN-ACC)Lead and mentor a team of RNs, care staff, and volunteersCollaborate with external providers to ensure seamless care deliveryMonitor and improve clinical programs and quality indicatorsChampion a culture of safety, wellbeing, and continuous improvementAbout YouAs a Care Coordinator, youll play a pivotal role in leading clinical care delivery for residents with complex needs. Youll oversee assessments, care planning, and compliance, while mentoring a team of Registered Nurses, care staff, and volunteers.
