Using trauma-informed and strengths-based approaches, you will deliver compassionate case management tailored to each clients journey.Youll also provide outreach support, collaborate with other services, and contribute to a positive team culture focused on safety, empowerment, and client-led care.Key responsibilities include:Develop, implement, and review client support plansAssess risks and assist with housing, health, legal, education or wellbeing goalsDeliver capacity-building support, such as life skills and community engagementMaintain accurate, timely records including NDIS-related documentationSupport clients moving into independent housing or transitioning between servicesWork collaboratively with the team and wider service network to support each clientYoull bring experience in case work or community services, a deep understanding of trauma and homelessness, and a passionate commitment to supporting women.You will have:Relevant tertiary qualifications in social work, human services, or related fieldMinimum 2 years experience in case management or similar support rolesExperience working with mental health, D&A issues, family/domestic violence and complex needsStrong knowledge of trauma-informed, strengths-based, and recovery-oriented practiceExcellent communication, time management, and crisis response skillsA belief in the inherent worth and potential of every womanOther requirements:Unrestricted Drivers LicenceDHS Disability Services Employment Screening / NDIS Worker ScreeningChild Safe Environments Training (current within 3 years)First Aid Certificate (or willingness to obtain)Even if you do not meet every requirement, we encourage you to apply.
