The Team:The Correctional Primary Health Services (CPHS) is part of the Tasmanian Health Service and provides primary healthcare to people in custody in Tasmania. We care for up to 800 people at a time, making us the largest primary healthcare provider in the state.Our staff practice every aspect person-centred care in all aspects of primary health. We provide assessments, practice acute and chronic care, mental health care, medication management, respond to emergencies and provide education and support to people in custody so they can maintain and improve their health.The Role:This is an exciting opportunity to shape the future of Correctional Primary Health Services.The role will involve Model of Care development across a multifaceted Statewide Service as well as outlining the care pathways for the patients moving through the custodial environment.You will will work with a diverse highly professional multidisciplinary team and the supporting executive to the Forensic Health and Correctional Primary Health Services.You will need:Significant experience in developing and implementing change management programs and/or experience working at a strategic level implementing modern and contemporary practices within a best practice framework.Proven liaison and consultation experience and the ability to develop and maintain strategic partnerships with a broad range of internal and external stakeholders to ensure the development and implementation of an integrated outcomes-focused project.Highly developed interpersonal, communication, decision making, conflict resolution and negotiation skills, and ability to foster support for change across a diverse employee base, with the proven capacity to motivate, develop and support staff through a positive change management process.Note: - Please refer to the Statement of Duties for all the selection criteria of this position to address in your application.Details of appointment:fixed term full time, day worker, working 76 hours per fortnight, commencing as soon as possible for a twelve-month period.notwithstanding hours to be negotiated with the successful applicant.Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.Salary: $128,102 to $134,832 per annum.
