Country SA PHN is seeking a dynamic and strategic Health System Integration Officer to champion initiatives that enhance primary care services across regional and remote South Australia.The RoleAs the Health System Integration Officer, you will:Establish Medicare Urgent Care Clinics: oversee the commissioning and operational support of new clinics, ensuring they meet compliance and quality standards.Support Strategic Projects: support the design, planning, and implementation of projects focused on clinical and stakeholder integration, ensuring alignment with national guidelines and Country SA PHN objectives.Enhance HealthPathways SA: guide the development and implementation of HealthPathways to promote consistent care and improve patient journeys.Support Integrated Services: design and establish services that foster a cohesive healthcare system in Country SA.Data-Driven Evaluation: utilise data to assess and improve primary healthcare services, focusing on integration and resource optimisation.This position is part of the Health System Integration and Innovation team. It is expected that this position will remain flexible to meet the changing needs of the organisation and the community.Key ResponsibilitiesCollaborate with internal and external stakeholders to co-design and implement integration-focused projects.Identify gaps and opportunities to enhance patient and clinician experiences.In collaboration with the Senior HSII Manager, support teams dedicated to embedding integrated care within the health system.Foster partnerships with general practices, allied health professionals, and community service providers.Promote best practices and continuous quality improvement in healthcare delivery.A full position description is available on request by emailing cstribley@countrysaphn.com.au or by clicking here.Why you should come work with us:Country SA PHN offers a great range of benefits to our employees, including:Competitive salary.
