Youll be responsible for but not limited to: Creating and updating individualised care plans based on face-to-face assessments Supporting participants with a model of care that focuses on their goals, independence, and wellbeing Ensuring care aligns with Aged Care Quality Standards and each participants Statement of Rights Monitoring service delivery and making sure participants have the right level of funding and support Management of Support staffAbout us:FOCUS Connect (formally known as Macarthur Diversity Services Initiative) is a registered, not-for-profit charity that offers practical help and support services to the disadvantaged and marginalized. Based on an integrated service delivery system FOCUS Connect''s programs are specifically tailored through 4 broad service areas: children, youth, family and agedWhat you will bring to the role: A strong working knowledge of the Aged Care Quality Standards (including the Aged Care Reforms - July 2025) Experience managing a caseload in a participant-focused environment Excellent communication skills and a genuine ability to connect with people Well-developed organisational and time management abilities Confidence in conducting assessments and developing personalised care plans A sound understanding of the Support at Home programWhat Youll Need: Bachelor''s degree in Nursing, Social Work, or related field (preferred). A tertiary qualification is efficient for the role.
