Based on an integrated service delivery system FOCUS Connect''s programs are specifically tailored through 4 broad service areas: children, youth, family and agedWhat you will bring to the role: A strong working knowledge of the Aged Care Quality Standards (including the Aged Care Reforms - July 2025) Experience managing a caseload in a participant-focused environment Excellent communication skills and a genuine ability to connect with people Well-developed organisational and time management abilities Confidence in conducting assessments and developing personalised care plans A sound understanding of the Support at Home programWhat Youll Need: Bachelor''s degree in Nursing, Social Work, or related field (preferred). A tertiary qualification is efficient for the role.
