The team provide specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. The role will be based at Marrickville Community Health CentreCommunity Mental Health Team (Core Team) is the mainstay of the community mental health system.Core Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.Consumers under the care of Core Teams will either have:Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.A period of treatment as recommended by evidence based practices.Ongoing care and monitoring for prolonged periods.The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.For more information, please view the Position DescriptionIdeal CandidateRecognised tertiary qualifications appropriate to profession.
