Our unique range of services include; Communities for Children, Family Health & Wellbeing, Childhood Development, Education & Support, Early Intervention Therapy Services, NDIS Support Coordination and Plan Management, Homelessness Support, Counselling Services, assistance to support victims of trauma, assault and violent crimes and Recovery Support Services.Due to increased consumer demand, we are seeking a Support Coordinator to service our consumers across (Dandenong, Casey and Cardinia regions) to join a skilled and dedicated team who are committed to providing high quality services and go above and beyond for our consumers to coordinate and implement the supports they need. Read more hereDesired Skills and ExperienceNow, let''s talk about this exciting opportunity!Working remotely and from our offices in Narre Warren or Pakenham (when required), you will connect consumers with service providers to help them access new services to meet the goals laid out in their NDIS plans. You will provide coaching and guidance to build the capacity of consumers and families to be able to self-manage their NDIS plans and funding as much as possible, empowering through choice and control and creating independence.This is a permanent Full-Time role (Part Time .8 may be considered) and a truly rewarding opportunity for someone with a professional background in disability, seeking a supportive team environment.*As Windermere is a not-for-profit organisation, employees are able to claim everyday living expenses up to $15,900 against their annual Pre-Tax Salary. Use the salary packaging calculator available here - - (Selection Criteria:Bachelor of Social Work or tertiary qualifications in relevant and related discipline desired, and/or equivalent professional/ industry experience3 years'' experience within disability services or related servicesDemonstrated knowledge of the Disability sector, relevant legislation including but not limited to the Disability Act 2006, National Disability Insurance Scheme Act 2013, Victorian Disability StandardsDemonstrated skills in independent problem solving, negotiating, creative thinking, self-initiative and capacity building ability for self and others.Demonstrated knowledge of mental health, family violence, child protection and drug and alcohol services highly regarded.Experience in Person Centered Planning, support coordination and facilitation.Demonstrated experience and commitment to supporting people with a disability to participate fully in their community.Sound knowledge of support coordination practice, assessment, family dynamics, community support networks and services available to people with a disability and/or their familiesDemonstrated ability to effectively manage use of time in fast paced complex service environmentAbility to meet set KPI''s on a fortnightly basis.Ability to use relevant technology including phones, computers and consumer data systems.Demonstrated ability to work effectively as part of a team as well as work independently, including office based or remote off-site locations.Willingness to promote and support practices that are inclusive, culturally responsive, safe and accessible.Current Victorian Drivers'' Licence.Willingness to undertake relevant pre-employment screening and checks - including NDIS worker screening check, Police Check, Pre-Employment Medical and Working with Children''s checkRight to Work in Australia e.g. Australian Citizen, Permanent Resident or Visa holder with full working rightsIf this job excites you, then click the Apply button to view the detailed and submit your application.
