Advanced Clinical Practitioner- Humber Virtual Frailty WardHours: 4 sessions- 2 sessions on a Wednesday and 2 sessions on a Friday- to be worked by one person or could be split between two people.As a skilled Advanced ClinicalPractitioner with the Humber Virtual Frailty Ward, you will be deliveringholistic care to frail and vulnerable patients across Scarborough, Whitby andRyedale.Based at the teams Community Hub inScarborough, you will work as member of a multidisciplinary team that includesAdvanced Complex Care Managers, a Senior Physiotherapist, a Specialist ClinicalPharmacist and Nursing Associates.
