In this vital role, youll contribute to innovative, evidence-based health promotion programs aligned with the goals and strategies of the South Western Sydney Local Health District and the NSW Ministry of Health. Primarily working within the Health & Wellbeing streamand supporting Early Years & Ageing projects as neededyoull be involved in the full cycle of preventative health program delivery, from planning and implementation to evaluation and sustainability. Youll also play a key role in supporting clinical and primary care referrals and delivering targeted programs around tobacco and vaping. Be part of a passionate team driving positive change for priority populations in our community.What You''ll Be DoingThe purpose of this position is to improve the health of people living in south western Sydney by contributing to Health Promotion programs of work in line with South Western Sydney Local Health District and the Ministry of Healths goals, principles, and strategies.The Health Promotion Officer will primarily work in the Health & Wellbeing stream and contribute to evidence-based and/or innovative programs of work for population health priorities. The Health Promotion Officer may also work on projects from the Early Years & Ageing stream as required.The HPO will primarily be responsible for working with the team todeliver all stages of planning, implementation, evaluation, reporting and sustainability of preventative health programs for priority populations.support clinical and primary care multi-risk factor assessment and referral to preventative health programs.deliver tobacco and vaping-related programs.Where You''ll Be WorkingSouth Western Sydney is rapidly growing and is a great place for young people and families to build a life. View our application guide for information on how to respond to criteria and improve your application.Tertiary qualifications in health or related field and demonstrated knowledge and experience in health promotion planning, implementation, evaluation and dissemination particularly relating to healthy eating, active living and/or tobacco and e-cigarette control.Demonstrated knowledge and experience of current health promotion theory and evidence-based practice.Demonstrated ability to establish and maintain partnerships with other staff and government, nongovernment and community organisations.Effective organisational, problem solving and team skills including the ability to work on a variety of projects simultaneously with minimal supervision.Applicants must possess an Australian C-Class driver''s license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
