This provides unmatched insight into a members overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.ResponsibilitiesConduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation following Matrix guidelines and protocols.Collaborate with physicians, case managers, social workers, family members, and other medical personnel.Coordinate with Primary Care Physician (PCP) on patient education and follow-up.Provide services in various settings such as home visits and skilled facility visits.QualificationsMasters Degree or equivalent experience with NP licensure.Current RN and NP licensure in the practice state, including prescriptive authority.Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.Current BLS, ACLS, or CPR certification.Preferred 1 year of NP experience; new graduates encouraged to apply.Strong computer skills and familiarity with medical record software.Excellent communication skills.Flexibility to adapt to change.Travel RequirementsTravel to patient locations as needed.Valid drivers license, automobile insurance, and ability to travel.Compensation ranges from $1,900 to $9,500 per month based on assessments completed. This is a PRN, flexible position with no benefits except 401(k) and voluntary benefits. Location options include home, SNF, and community environments in Eastlake, Ohio.
