Senior Case Manager Home Care | Lead Complex Client Support & Mentor Others South Brisbane | Permanent Full Time | Excellent Salary Not-for-Profit Benefits Are you an experienced Case Manager looking to step into a more senior, impactful role? Youll handle a smaller caseload of high-needs clients while mentoring and guiding a team of Case Managers. What You''ll Do: Manage a portfolio of complex Home Care Package (HCP) clients Support onboarding, coaching, and ongoing development of other Case Managers Act as a senior point of contact for escalation and problem-solving Collaborate with internal clinical, finance and rostering teams to ensure seamless care delivery Uphold quality, compliance, and best-practice service coordination under Aged Care Quality Standards About You: Proven experience in case management within the aged care or community care sector Confident in managing complex client needs and navigating diverse situations Enjoy mentoring and supporting team development In-depth knowledge of HCP funding and consumer-directed care models A clinical background (RN, EN or Allied Health) is desirable but not essential Compassionate, organised and solutions-focused Why Join?
