Your Opportunity Are you passionate about making a difference in the lives of young people? As a Youth Case Manager within the Housing and Support link Service, you''ll be working on the front lines from the Mater Hospital Young Adult Health Centre in South Brisbane, helping young individuals who are homeless or at risk of becoming homeless while they receive treatment or support. Youll work closely with these young people and their health and community networks to break down barriers to stable housing. Your mission? To find and secure housing solutions that support their recovery and help them develop the skills they need for long-term tenancy success. Your key responsibilities will be to: Support young people who are homeless or at risk of becoming homeless to stabilise their housing arrangements. Work with participants to create individualized support/case plans including referral. Assist clients in the process of transition out of the service. Conduct group case management sessions or group work programs for clients. Engage and participate in relevant community-based meetings. Complete a range of administrative duties and internal/external reports. Ensure all risks/ incident reports / safety concerns are documented. Requirements for Success: Relevant degree with at least 1 years experience or Associate Diploma with relevant experience or less formal qualification with substantial years of relevant experience. Demonstrated experience supporting young people with high and complex needs. Knowledge and understanding of Child Protection legislation and child protective concerns. Demonstrated crisis, case management, referral, and advocacy skills. Demonstrated professional approach to the role including maintaining professional boundaries. Strong stakeholder management skills. Strong organizational skills and the ability to manage time effectively. A sound level of computer literacy including proficiency in Microsoft Office packages. Required to get vaccination for Hepatitis B, Mumps, Rubella, COVID 19, Measles, Whooping Cough, Chicken Pox and Tuberculosis as per the Biological Safety Policy. Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
