Add expected salary to your profile for insightsAbout the roleWe are seeking a passionate and experienced Support Coordinator to join our team at Supports For ME Pty Ltd on the Sunshine Coast, Queensland. As a casual Support Coordinator, you will play a vital role in assisting our NDIS clients with their needs to access the services and support they require to live independently and with dignity.What you''ll be doingCoordinate and monitor the implementation of client NDIS support plans, ensuring all services are delivered as per their individual requirements and assist them to maintain these supports in order to achieve their goalsLiaise with clients, their families, and a wide range of service providers to facilitate the delivery of holistic, person-centred careLiaise on behalf of clients to ensure their needs are understood and met by support servicesRegularly review client progress and make adjustments to support plans as requiredMaintain detailed case notes and records in accordance with organisational and legislative requirementsParticipate in team meetings and contribute to the continuous improvement of our service deliveryProvide care coordination supports to the wider team and ensuring required documentation is completed in line with legislation requirements for NDIS participantsEngaging in local network and stakeholder meetings to stay informed of services available and promote our organisationWhat we''re looking forPrevious experience as a Support Coordinator is essentialQualifications in disability services, mental health or community work highly desirableExperience in case management, reporting requirements for NDIS and stakeholder liaison is keyExcellent communication and interpersonal skills, with the ability to build rapport and advocate effectively on behalf of clientsStrong organisational skills and attention to detail to ensure accurate record-keeping and timely service deliveryDemonstrated understanding of the NDIS and the ability to navigate complex funding, service arrangements in this evolving environmentEmpathy, patience, and a genuine commitment to improving the lives of people with disabilitiesWhat we offerAt Supports For ME, we are committed to providing a supportive and flexible work environment that allows our employees to thrive.
