Visit us today and check out www.novascotia.com to see why more people from across the globe are moving here.About the OpportunityEarly identificationof patients requiring complex discharge planning case management through high-risk screening criteria, bullet rounds, and referralsAssessmentof patients medical, functional, cultural, and psychosocial needs, family issues/dynamics, and extent of community resources in light of input/assessments from the multidisciplinary pod and community partners re: identified needs and care options to facilitate the transition through care continuumPlanningwhich is a dynamic process and involves continuous review and communication of potential plans and timelines in partnership with the multidisciplinary team, client/SDM, family, and community agencies/facilitiesFacilitationof safe patient discharge throughongoing assessment, case coordination, and development of appropriate discharge plans.Included but not limited topatient/family education; referral and completion of application to specialized treatment programs; coordination of family meetings; coordination of legal/financial capacity or SDM issues. Participation in bullet rounds, Joint Discharge Operations JDO- meetings, and (where appropriate) weekly multidisciplinary team rounds.
