Transition Compass Coach (Co-ordinator) Employment Type: Full - time (35 hours a week) Duration: 2-year Fixed Term contract Remuneration : $100,354-$107,054 base, plus 17% superannuation and leave loading Work rights required: Visa sponsorship is not available for this position. Candidates must hold full rights to work in Australia to be considered for this position. Location: Randwick, Sydney with occasional travel to Westmead Hospital (approximately one day a week with an ability to be flexible) Discipline of Paediatrics & Child Health The discipline of Paediatrics & Child Health leads the delivery of educational and research activities aimed at the maintenance of child and adolescent health from the neonatal period until 18 years of age. Details of the role The primary purpose of this role is to support the clinical trial of Transition Compass (TC), an intervention designed to support adolescents and young adults with chronic health conditions transitioning to adult healthcare. The Transition Compass Coach (TCC) is a pivotal component of the intervention, providing one-on-one support to trial participants requiring extra assistance with navigating the adult healthcare system. The TCC will also provide effective administrative and research support to ensure the clinical trial is executed in accordance with strict research and ethical guidelines. This is a multi-site role encompassing two locations, the Sydney Childrens Hospital Randwick and the Childrens Hospital at Westmead. Some key skills required: A tertiary qualification in nursing, psychology, allied health, or a relevant health science discipline. Demonstrated understanding of clinical care and the Australian healthcare system, including pathways between paediatric and adult services. Ability to engage and build rapport with adolescents and young adults; assess their needs, identify barriers to healthcare access, provide tailored one-on-one support, and facilitate peer support sessions. Ability to work collaboratively with the Transition Compass research team across local and external sites, including strong interpersonal communication, a proactive and professional attitude, and accurate, timely record keeping. Ability to confidently use the electronic medical record (eMR) system and liaise with paediatric hospital clinics to identify eligible participants. Ability to communicate sensitively with young people to complete recruitment in line with the study protocol. Ability to onboard participants to the digital platform, monitor their progress, and escalate support when needed in accordance with trial protocols. Ability to collaborate with existing transition services to align support and referral pathways. Please refer to the position description for full details.
