This project will see significant changes across key priority areas positioning Outcare for sustained success into the future.The Role:The Case Manager plays a key role in supporting participants of the Start Court program, their families, and carers by applying a person-centered approach to recovery. You will assist participants in their journey toward community reintegration by providing tailored support that addresses their unique needs, promoting overall well-being, and facilitating their recovery.A significant aspect of the role involves attending court to advocate for participants, provide updates on their progress, and ensure the court has a comprehensive understanding of their circumstances. You will work closely with a multidisciplinary team, including Peer Workers, Family Carer Peer Workers, Community Corrections, AOD Officers, Forensic Psychiatrists, Clinical Psychologists, Social Workers, and Clinical Nurse Specialists, to deliver holistic and coordinated support.Outside of court, you will offer psychosocial support in crucial areas such as accommodation support, accessing education and training, and facilitating the groups that Outcare offers to the participants of the program. Your efforts will focus on breaking down barriers to recovery, connecting participants with essential services, and helping them achieve long-term stability and successful reintegration into the community.The Responsibilities: Apply a person-centered approach to support participants, families, and carers. Foster recovery and well-being through skill-building, activity planning, and community outreach. Conduct risk assessments and implement intervention strategies to ensure safety. Facilitate psychosocial support in areas such as accommodation, education, training and group facilitation Actively participate in court proceedings and multidisciplinary meetings. Complete administrative tasks including documentation, rostering, risk assessments, justice stars in line with reporting in a timely manner that meet contractual requirements Collaborate with legal stakeholders, health professionals, and Peer Workers. Address broader participant needs to support holistic recovery and integration. Regularly attend court sessions as required, advocating for program participants'' needs and progress.What Youll Need: Tertiary qualification in one or more of the human services fields (Cert IV in community services, Social Work, Psychology Degree etc) Previous experience supporting people and their families in a justice setting. Knowledge of issues and barriers facing individuals interfacing with the justice system in Metropolitan Perth. Previous experience in working in a case manager role supporting individuals with complex issues, AOD, justice involvement and mental health. High level written, oral and interpersonal communication skills that can be used to engage a wide range of people including program participants and their families, internal staff members, government agencies and other stakeholders. Demonstrated strong organisational and analytical skills and ability to plan and prioritise workload in order to achieve expected outcomes and meet deadlines. Experience in working successfully with people from diverse cultural, linguistic and religious/spiritual backgrounds, low-socio economic and disadvantaged groups, and clients with a history of justice involvement. Knowledge of the current National Standards for Mental Health Services. Demonstrates cultural competency in working with Aboriginal and/or Torres Strait Islander People. High-level proficiency in the use of technology including Microsoft Office - email, Word and Excel.Other Employment Requirements: Must hold Australian Citizenship or hold a visa that allows you to work in Australia National Police Check. Prior Justice involvement should not prevent any application. Proficiency in using technology, including Microsoft Office and email. Working with Children Check.
