Rehab at Home operates within the Division of Western Health at Home. Rehab at Home is a subacute bed substitution program that provides intensive rehabilitation to patients discharged early from hospital. The program has recently expanded, and now accepts patients with non-neurological diagnoses where rehabilitation is indicated to support recovery.The successful candidate will manage a complex mixed caseload of clients with neurological, orthopaedic and musculoskeletal conditions.
